When remitting online, the rate and charges are shown before I confirm. With a physical form at the branch, how can I know the markup and fees in advance?

You should ask bank staff markup fees and processing fees before you submit form. I don’t process unless i have clarity on charges.
Only grey area in offline is the rate at the time bank decide to process your transfer. Like yesterday lowest was 95.38 in the morning, highest was 95.69 at the closing; so depending on what time bank process it can vary in this range. I luckily got 95.43
