hi i have filed and received the acknowledgement for the FY 2009-10.I have to pay tax of Rs.1380.how do i pay the amount and are there any forms to be filled to notify IT department.what are the penalty charges for this per month?

U have to pay tax immediately of Rs. 1380/- Plus Interest thereon @ 1 % per Month(U/s 234B) from March, 2011 to May, 2011 and file your revise return online if u filed original return online earlier. Otherwise u have to contact your assessing officer immediately.
For filling online revise return a penalty for Rs. 5000/- may be imposed by your assessing officer.
