Format & Procedure to get Certificate for 80DD

My father has Hearing impairment; we went to Govt Hospital to request for the Disability certificate. They are calling that as Physically Handicapped Certificate!!! (The process for getting is really complicated.) Is that the same as Disability certificate required for IT exemption. As I understand, The Disability certificate need to be in the format as specified by "". But after going there, I was totally confused, it has entirely different format; with photo attached to it etc.

What is the actual procedure to obtain certificate to claim 80DD exemption? Is it enough to get it signed on the form 10-IA (as above), with authority as specified in that? or is it OK to submit the Physically Handicapped Certificate as the proof for IT exemption?

Certificate should be in same format as specified in 10-IA.

You just need to declare exemption for same in income tax return filing. Proof/certificate copy need not to be attached with income tax return.

In case IT department needs it, they can ask for disability certificate later.
