regarding withdrawal of epf

hello, my father who has retired from his job 5 years ago, went to withdraw his epf, he submit the form along with the copy of the bank account pass book. but he was ask to submit the cheque leaf. we are settled in kerala and his epf account is at mumbai.. Is it required to submit the cheque leaf? can anybody tell me whats the actual requirement?

In order to get withdrawal amount directly transferred to holder's account, a cancelled cheque needs to be attached with EPF form.

Now-a-days no EPF withdrawal form is accepted without this as all settlements are being done through NEFT/RTGS.

Also don't forget to mention mobile number on top of form, to receive status SMS.
